Looks quite good, yes! I couldn't find the details about which parts you ordered and where you ordered from - just remember I read that it was quite cheap. Have you got the links handy?

Lens is this one. Simply taped on the camera's lens. The camera is MD80 action camera. There seems to be lots of similar looking and MD80 named cameras. Cheapest are about $10, mine is from Finnish web store verkkokauppa.com, 36 euros. This shoots 720x480 resolution. This has plastic body, 35 g with the lens. There is heavy aluminum ones too, I'd say plastic body version is better for a head cam. I had to buy 8 Gb memory card (class 6 card, 19 euros), original one wasn't enough for one hour races. Battery should last over an hour in summer temperatures, I managed to shoot a wall for 80 min indoors before it ran out of power. About ~ 65 euros at the end, one could cut it down by buying cheaper camera & mem card, lens wast the cheapest out there either. But you never know about the quality, I tried to play save here.

None of those. It is not 808 car key style camera, it is MD80 one.

Could you upload somewhere raw file from camera? People says that 80 and 808 generally is same product in different shape. Quality difference between versions is huge. Some # has big % of missing frames, some # has more motion blur due harder compression.....

This makes 22050 Hz audio, so it's #3 or #6. But this has blue& red leds and this makes no time stamp out of the box. I haven't done tag.txt thing and will not do because I am not sure can I take time stamp away after adding it. But I guess this none of those or a new version of 3 or 6.
