To where should the entry be sent? How much is it? What information must accompany it? The NEOC website has no data.

Yeah, I know.
I sent them a reminder to update the website.
Entry: send a check for $20 made out to NEOC to me at
74 Richards St, Dedham MA 02026.
Include your T shirt size, epunch number,and your O-age.

Attackpoint says today is the deadline; NEOC's web site says 11/7 is the step-up in reg fees.
