I would help you, but for some unknown reason MS Word freezes up everytime I tried to save that file as another format.
Maybe someone with better luck can do it, or I'll think of something else.

yes, fee is $23/day. USOF/IOF member gets a $3/day discount. Also E-punch rental is $2/day.

Well, in case that doesn't work for you, I just sent you a pdf version of the form. So check your email. Hopefully your computer can read it without problem.
I got the form out half ok on MS word, but the sprint line was mangled at the bottom. I asked Mr. G and got a prompt early morning response: Add $10 and you are registred for the sprint, too. There must be a mad rush to the P.O. today. Another 0.6k to post, if I run it.
Actually, you can move the sprint box to an open area - say to the R side - and print it out, circle Y (of course) and mail in.
